What's wrong with me? I HURT! I did have an MRI.

I fell while getting back out of my truck. I suppose it's about 4.5 to 5 feet in height. I got the MRI report.

First, the report says:
There is mild deformity of the T11 superior end plate incompletely included at the upper edge of the images, possibly remote superior end plate fracture.

Secondly, since the MRI report also states:
At L4-L5, there is a broad-based central disc protrusion with partial annular fissure causing asymmetrical narrowing of the left subarticular region and mild compression of the left L5 nerve root seen on axial Image 8. There is mild right subarticular narrowing at this level without definite compression of the right L5 nerve root. Very minimal facet arthropathy is present at the level.

I HURT. My right hip was the first pain I noticed that didn't go away after about 10 days after my fall. Then my middle back tensed up. I ignored it thinking it was stress ( I drive an 18 wheeler). I kept working until about 6 weeks ago. My lower back started hurting. Now my right leg goes numb. My left leg goes numb now as well after I walk about 2 blocks. I don't sleep because once I go to sleep and get woke up from hurting or noise I can't go back to sleep. Even ambien cr isn't helping. I got horrid spasms in my back last week, and I couldn't hardly walk because of them. My legs sweat at night as well.

The insurance company sent me to a pain clinic. I start steroid injections on the 8th of Jan. They also sent me to an orthopedic surgeon who examined me for all of five minutes today then told me I was fine, and I would get better in time. I asked him for a muscle relaxer that would work, and he said he doesn't prescribe them. I'm not so sure I'm fine. I can't even stand in one place for 15 minutes without going numb. It feels like my back is going to break.

Should I insist on seeing a neurologist? I don't want surgery, but I also can't live like this! I'm taking only zanaflex and mobic. My ambien is gone, and I didn't bother getting a refill. I have no pain meds unless I go to the ER.

Is this an ortho problem or neuro? I'm scared of nerve damage :(
