For the past few years I have had issues with my neck/back. I am having severe pain in my neck and upper back between my shoulder blades. I have pain and numbness in both arms, the right more than the left. I also am having severe lower back pain that begins in the middle of my lower back and goes down my hip and into the back of my right leg with numbness in my right foot and big toe. I have began to be clumsy and have fallen several times. In the recent past I have had: epidural shots, facet injections, radiofrequency and cryoblation. These helped the pain, but for only short periods of time. I have recently changed doctors after moving to a new state and now I am becoming confused about his treatments/lack of treatments. I just completed an MRI on the cervical and lumbar areas (after insisting) and hope that someone could give me a subjective opinion.

There is reversal of cervical lordosis with moderate disc space narrowing at C5-6; mild an C4-5 and C6-7
At C4-5, there is a small left paracentral disc ostephyte complex slightly indenting the thecal sac and almost touching the cord. There is no significant neuroforminal impingement. At C5-6 there is diffuse disc osteophyte complex with spinal stenosis. The AP canal diameter is narrowed to 8mm. There is neuroforminal impingement right greater than left.
At C6-7 there is a minimal left lateral recess disc bulge with slight left neuroforminal impingement. At no other levels are focal disc protrusions or spinal stenosis. The cervical cord is normal in size and signal.

Impression: DDD

Vertebral alignment, body heights and bone marrow signal pattern are normal. The conus terminates in normal position and is normal in size and signal. Mild disc space desiccation is present at L5-S1, L1-2 and L2-3.
At L5-S1 there is mild facet arthritis with a minimal central disc bulge. At L2-3 there is a small right foraminal disc protrusion minimally indenting the thecal sac. There is really no significant neural foraminal impingemnt. DISC FRAGMENT DOES EXTEND SLIGHTLY BEHIND THE l2. aT l1-2 THERE IS A SMALL CENTRAL DISC PROTRUSION WITHOUT SIGNIFICANT MASS EFFECT.
