Could someone help me figure out what this EMG result mean?

I am trying very hard to figure out what this emg report(results) mean but is unsuccessful. I am kindly asking if someone can give me a hand in what it means?

There are 2 pages of the writting and 2 pages of some sort of graph and data. I upload it onto photobucket and all 4 pages is on there. I wasnt quite sure how to post it on the forum.
