bilateral facial pain

hello,

i am 33, female, healthy. last august of 08 i had a sudden onset of pain that began in a tooth and spread to both sides of my face in one week. i have had extensive testing including two thin slice mri/mras to rule out MS and TN, a ct scan of my face, blood work to rule out auto immune disorders. everything has come back normal. i have no dental problems.

i have had no trauma or injury to my face. no illness, no shingles.

i have no numbness. however, i do have sensory disturbances, like "ants crawling" across cheeks, and pins and needles are elicited when i tap above my eyebrows. it radiates to my scalp. my cheeks are always aching, my forehead is tight and stinging, my scalp burns. it is bilateral and simultaneous. cold air evokes pain. heat does not affect pain.

i would like to know what other tests i could have to help diagnose my problem. should i have trigeminal evoked potentials? what about an auditory brainstem evoked potential?

i take nortriptyline at 50 mg at night and lyrica 75 mg twice a day. i do not know how well they work as i am in pain everyday.

i would appreciate any feedback or suggestions.
