If small fiber neuropathy doesn't come from peripheral neuropathy, what does it come from? Could it have been prevented? What does it progress to?

Small fiber neuropathy is a subtype of peripheral neuropathy....it usually goes with autonomic neuropathy.

If small fiber neuropathy doesn't come from peripheral neuropathy, what does it come from?

I was told my EMGs were normal. I assumed that meant I don't have peripheral neuropathy.

I am going to Greenwich, CT to a specialist in autonomic neuropathy there this Friday. I asked my doctor to send limited amount of info for I feel I would like someone to start from scratch - no previous misconceptions to view since none of my doctors seemed to agree on much of anything. What can I do to give myself the best chance of presenting my case?
