memory loss/ help

What might cause a person to forget something so quickly. Example, my son knows to pee in the toilet but will just stand there and not even care or realize he just peed on the floor. He is waaaayyy to old to do this (8). It reminds me of a dog. It is soooo sad and confusing. Is there a test you would recommend?

Does he have any concentration issues...e.g. in school?
Is he dry at night?
